United Healthcare Received $4.5M Funded 5-year Accountable Health Communities Award In Hawaii

United Healthcare recently received a $4.5 million funded 5-year Accountable Health Communities award from the Centers for Medicare & Medicaid Services (CMS) to improve social related health services in Oahu, Hawaii. The program seeks to partner with Waianae Coast Comprehensive Health Center, Kalihi-Palama Health Center and The Queen’s Medical Centers to bridge the gap between clinical care and social services for eligible Medicare and Medicaid patients.

The Accountable Health Communities Model enforced by UnitedHealth care will take care of health-related social needs of the eligible recipients of Medicare and Medicaid like employment, decent transportation, safe housing, and financial stability. In addition to the above funding by CMS, UnitedHealth care is additionally investing $6.4 million for the initiative.
Katherine Keir, UnitedHealth care Hawaii state director stated “Social barriers, such as housing instability and quality, food insecurity, personal safety, and lack of transportation and affordable utilities, affect people’s ability to maintain their health and well-being” as per reports.
In addition to the above local health care partners, the program also includes a team of navigators and an advisory board of local stakeholders to overlook the execution and making sure that the patient needs are met and taken care of.
